1) In the Disabled Health Board Report received from the public hospital or university hospital, the phrase "Spiritually Fully Dependent" should be included. Writing the phrase "Partial dependent" in the report includes negativities in terms of free institutional care.
2) Income per capita in the residence of the disabled person; less than 2/3 of the net minimum wage.
3) Guardianship: If there is a mental definition in the disability health board report, he/she should obtain the Dispatch Form (Document Regarding the Case of Custody), which identifies that he/she has filed a lawsuit to appoint a legally competent Turkish citizen (if possible, his/her relative) to the Civil Court of Peace in the residence of the disabled person.
4) Documents that should be in the file to be prepared:

-Health Board Report (original)
-Original of the guardianship decision
-Photocopy of the disabled person's identity card
